Isle of Evil
How do you start this quest? The dialog Rapanaio gives on Zeal is different than what is shown in Tibia, and I can't figure out how to make the quest line start.

Note: I can go to the actual isle just fine, but can't seem to do the quest itself.

Is there any way to get to "The Cave" which is part of the 5th mission? That's really the only thing I wanted; access to a reliable pirate location.

I'll look into setting up access for it. Can you not simply hunt in Nargor or Yalahar Pirates?

Nargor requires completing two different quests to access. I've added them to my list and will get to them; the Isle of Evil cave was appealing because it was less quest work to reach the pirates, and they seemed (based on the wiki info) to be heavily concentrated in sufficiently high numbers, whereas Yalahar has a bunch of other things mixed in.

If I am misinterpreting Yalahar's pirate area, or if the Meriana and Shattered Isles quests are not required to reach Nargor, then I'll happily go to one of those places.

On that note, it would be good to add the quest differences to the list of differences FAQ on these forums. I can think of a few (the Isle of Evil one and the Explorer quest line that is usually required to reach Calassa), and there are probably several I'm not aware of.
